Activity Description

Screen for perinatal mood and anxiety disorders (PMADs) and substance use disorder (SUD) in pregnant and postpartum women, and screen and refer to treatment and/or refer to appropriate social services, and document this in patient care plans.

Objective

Improve outcomes by providing appropriate mood, anxiety and substance use disorder screenings and treatment during the perinatal and post-partum periods.

Suggested Documentation

Evidence of documented screening for perinatal mood and anxiety disorders (PMADs) and substance use disorder (SUD) in pregnant and post-partum women with evidence of treatment and/or referral to appropriate social services and document in the care plan. Include the following elements:

1. Identification of perinatal and postpartum population - Report from the electronic health record (EHR), qualified clinical data registry (QCDR), clinical registry or other system demonstrating the tracking of perinatal and post-partum women; AND

2. Evidence of perinatal mood and anxiety disorders (PMAD's) and substance use disorder (SUD) screening - Report from EHR, QCDR, clinical registry or documentation from medical record chart showing regular practice for screening for PMAD's and SUD; AND

3. Evidence of appropriate treatment and/or referral documented in care plan - Report from EHR, QCDR, clinical registry or documentation from medical chart showing appropriate treatment and/or referral for PMADs and SUD.
