Make tools supporting self-management available to patients, thus contributing to improved patient engagement, adherence to treatment, and health outcomes.Suggested Documentation
Documented use of tools to assist patient self-management support assessment. Include both of the following elements:
1) Evidence of tools used – Documentation in medical record or electronic health record (EHR) showing use of tools designed to assist patients in assessing their need for support for self-management. Tools may include but are not limited to Patient Activation Measures, which assess an individual's knowledge, skill, and confidence for managing one's health and healthcare, and How's Your Health, which helps patients create an action plan; AND
2) Results of assessment and care plan – Documentation in medical record or EHR showing results of patient's self-management support assessment and care plan for addressing support needs.
