Activity Description
For at-risk outpatient Medicare beneficiaries, individual MIPS eligible clinicians and groups must attest to implementation of systematic preventive approaches in clinical practice for at least 60 percent for the 2018 performance period and 75 percent in future years, of electronic medical records with documentation of screening patients for abnormal blood glucose according to current US Preventive Services Task Force (USPSTF) and/or American Diabetes Association (ADA) guidelines.
Objective
Screen more patients at risk for diabetes.
Suggested Documentation
Evidence demonstrating the implementation of an abnormal blood glucose screening program focused on at-risk populations. The population/s for this activity are to be defined by the eligible clinician and might include (but are not limited to): patients over a certain Body Mass Index, patients with a family history of diabetes, or patients of an atrisk race or ethnicity. Include both of the following elements:
1) At-risk population identified – Total stratified number of Medicare patients at risk for abnormal blood glucose; AND
2) Percent of population screened – Total number and percentage of at-risk population screened for abnormal blood glucose as outlined by the US Preventive Services Task Force and/or American Diabetes Association guidelines.
